HOMECOMING DANCE TICKET SALES! The Golden Age of Hollywood homecoming dance will be Saturday, September 22, from 7:00-10:00 p.m. at the Community Choice Credit Union HyVee Hall. Tickets are on sale Monday through Thursday in the front hall by the auditorium and are $10 each. Purchase times are 7:45-8:20 a.m. AND 3:25-4:00 p.m. BUY YOUR TICKET EARLY-don’t stand in the long lines on Thursday! Guest forms (invited students from other schools) must be turned in when purchasing the both tickets.
Picture Retake Day is September 26 from 7:45 a.m. – 2:00 p.m. in the south commons. All students, including seniors must have their picture taken as a school record.

Senior Year
Important information for seniors and families on senior year deadlines & activities!
Silver Cord – Community Service
Student Parking Permits – Available to seniors and juniors at Roosevelt. Permit forms are available in the at Back to School event and in the main office. Students complete the form, including make, model, year, color, license plate number, and return with $20. Students must provide a current valid driver license and insurance card for the vehicle to complete the process. One per student limit, 200 available. Sold out for 2018-19.
Supply Lists
Only 9th grade students have a pre-selected supply list.
Grades 10, 11, & 12 do not have specific supply lists. Students are expected to bring school supplies daily such as their planner, pens & pencils, a notebook, spiral notebooks, or pocket folders to collect and organize their work, blank paper, needed textbooks and a free reading book. Teachers will inform students of specific supplies as their courses develop. Additional supplies are common for elective classes.

Request for Transcript
OFFICIAL TRANSCRIPT
A copy of your official transcript must be sent directly from the high school to a college, vocational school or employer to comply with application procedures. This completed form needs to be returned to the registrar’s office along with a stamped, addressed envelope (stamps needed will be according to size and weight of the envelope – usually two first class stamps are needed). Please leave the return address BLANK. If there is a deadline for the transcript, please see that the registrar office has this information at least five days before the deadline date, allowing enough time to get it there by mail. After five transcripts, there is a $3.00 charge per transcript. Transcripts will not be faxed or Express mailed.
UNOFFICIAL TRANSCRIPT
An unofficial transcript may be requested with at least a day’s notice to take with you when moving to a new school. This will assist the new school with assessing your credits and placing you in the correct courses immediately. An official request will still need to be made from the new school to receive the official records from Roosevelt.
